Cancellation Policy
Cancellation policy: all patients or clients must cancel 24 hours prior to their appointment. If not cancel within 24 hours, there will be a $75 charge.

We cannot draw blood if we do not receive consent to do so from our patients. This is a friendly reminder to fill out New Patient Intake Forms promptly.Your appointment will be canceled if we do not receive forms in a timely manner.
